Hello All,So, eto yung exeperience ko kanina sa PRC BUTUAN.Orig. Inally sa Cebu ako nag take at sa Butuan ako nagpa initial register at succesful naman. Eto yung requirements (Butuan)1. Printed out Oath Form (you can print it once you successfully registered sa Initial Registration)2.
3 Colored pictures with collar with name tag (See attached photo)3. 2 Documentary stamps (much better if u have those with you kesa sa PRC ka bumili kasi baka wala dun)4. Photocopy of one Valid ID (back to back muna para alang sabit at preferably yung ginamit mo nung nag take ka)5. Letter of Request kung d ka same ng PRC kung san ka nag take at nag initial register for oath taking (see format on the second photo)6. 1050 pesos for the payment. Wala na akong binayarang iba pero depende narin siguro sa site.Note: hindi na hiningi ang NOA at Cedula pero dalhin mo narin para sure.Tanong ko.
